If your child has a documented allergy, we request your assistance with the following:

Meet with your child's physician to complete an Allergy Action
Plan: Allergy Action Plan

Please return the Allergy Action Plan to the school nurse and speak
with the nurse before school begins, to explain your child's allergy,
symptoms and treatment.

Properly label and deliver emergency medications which may need
to be administered in school.

Prepare your child by reviewing and rehearsing the Allergy Action
Plan.

Keep the school nurse up to date on any changes.
